Resumo: A female patient, 82 years old, presented with an asymptomatic verrucous leukoplakia on then left lateral border of the tongue, with 3 cm in in the largest diameter, with an unspecified evolution. No risk factors were identified. An incisional biopsy was performed, which presented as an anatomopathological report of nonspecific chronic inflammatory process, negative for malignancy. Clinical diagnosis was of proliferative verrucous leukoplakia (PVL). The patient underwent total resection of the lesion under general anesthesia, with confirming previous anatomopathological diagnosis. During the 2-year postoperative follow-up, the patient presented recidive of the lesion, and a new incisional biopsy and subsequent resection under general anesthesia with a report of squamous papilloma, negative for malignancy. The patient has been in clinical follow-up for 2 years without signs of recurrence. This case demonstrates the importance of permanent clinical follow-up of patients with PVL, as recurrence is a frequent occurrence before malignant transformation.
