Resumo: Peripheral ossifying fibroma is a reactive lesion that originates from aggressions to the gingival tissue, which predominantly affects the anterior maxillary region. A 27-year-old male patient presented gingival swelling in the anterior mandibular region. Clinical examination revealed a nodular lesion measuring 1.5-centimeter diameter, painless, fibrous without bleeding, delimited in the free marginal gingiva between the lower central incisors, with 6 months of evolution. After surgical removal of the lesion, histopathological analysis revealed a dense fibrous connective tissue, well vascularized and cellularized, associated with focal areas of formation of mineralized material, organized in basophilic globose structures, establishing the diagnosis of peripheral ossifying fibroma. The patient remained without symptoms and with no signs of recurrence after 36-month follow-up. We conclude that treatment should include total removal of the lesion and probable causal factors, minimizing the tendency to relapse.
