Resumo: The clinical importance of the anatomical presence of canalis sinuosus (CS) converges in studies that demonstrate that this anatomical structure could simulate a lesion at the root level and induce the professional to perform an inadequate endodontic treatment, injure nerve branches during placement of dental implants or even cause paresthesia after a surgical procedure. A 23-year-old female patient presented, in a routine radiographic examination for surgery on impacted molars, a radiolucent image consistent with a root resorption lesion in the middle root third of the upper left lateral incisor. After pulp sensitivity tests and a Cone Beam Computed Tomography (CBCT), it was found to be a canalis sinuosus mimicking an inflammatory root lesion. Thus, it is demonstrated that anatomical knowledge is imperative during diagnosis, to avoid mistakes in radiographic interpretation and to prevent misguided treatments.
