Resumo: Osteoma is a benign osteogenic tumor rarely found in paranasal sinuses. A 85-year-old woman, with diabetes and Alzheimer's disease, was referred to our service for evaluation of painful swelling of the middle third of the face with about five years of evolution. Extraoral examination revealed a periorbital swelling with hard consistency on the left side. Computed tomography of paranasal sinuses was then requested. The sections showed a hyperdense image into the left ethmoid sinus, suggestive of an extensive osteoma. The tumor developed from the ethmoid bone and was compressing the walls of the orbital cavity and maxillary sinus. In addition, it showed a large extension to the nasal cavity on the same side. The patient underwent lateral rhinotomy for tumor excision. Histopathological examination showed an osteoma constituted by mature lamellar tissue. The patient is being monitored by our interdisciplinary team and a one-year follow-up showed no signs of tumor recurrence.
