Resumo: The range of microscopic features of the salivary gland tumors can lead to misdiagnosis. A 63-year-old woman, without comorbidities, was referred to our service for evaluation of a lesion on the hard palate. The patient showed a previous pathology report of the lesion, which indicated the diagnosis of Adenoma Pleomorphic. Extraoral examination showed no abnormalities. Intraoral physical examination revealed an ulcerated lesion, measuring approximately 1 cm, on the left side of the hard palate. Imaging tests were requested and showed no changes in the region of the lesion. Due to the clinical features of the lesion, which could indicate malignancy, a review of the slides from the previous biopsy sample was then requested. In the review, malignancy microscopic features were found and led to the diagnosis of an Adenoid Cystic Carcinoma. The patient was informed about the diagnosis and referred to oncological treatment with our interdisciplinary team.
