Resumo: Keratocyst is a developmental odontogenic cyst, with a small predilection for males. The most affected region is the posterior mandible. As for the radiographic features, it shows a radiolucent area with well-defined regular radiopaque margins. It can mimic a nasopalatine duct cyst or periapical cyst if it occurs in the midline anterior maxillary region. A 48-year-old man was referred for evaluation of a radiolucent lesion between the roots of teeth 11 and 21. On clinical examination a discreet swelling was observed in the vestibular region, near the apex of teeth 11 and 21. CT scan showed an independent lesion of the nasopalatine duct and the main diagnostic hypothesis was periapical cyst. As no regression occurred after endodontic retreatment, the lesion was surgically removed, and microscopic analysis provided the diagnosis of keratocyst. This case emphasizes the importance of correct diagnosis and follow-up of intraosseous lesions.
