Resumo: A 16-year-old male boy was referred to an oral and maxillofacial surgeon after an asymptomatic well-defined radiolucent lesion was identified in the right mandibular body found on a panoramic radiograph. Extraoral physical examination revealed no abnormalities, but an intraoral exam revealed an increase in volume, that was tough in palpation, in the area of teeth 42 and 43 extending to the vestibule. Tomography revealed fenestration of the vestibular and lingual cortical. Under local anesthesia, an aspiration revealed no content, leading to an excisional biopsy with complete enucleation of the lesion followed by ostectomy. Histological analysis revealed fusocellular proliferation rich in giant cells, with hyaline stroma and old bleeding areas , which could also suggest brown tumor of hyperparathyroidism; however, this was excluded with a normal parathormone dosage. The final diagnosis was central giant cell granuloma. Two years of radiological follow-up showed no signs of recurrence.
