Título: PROGNOSTIC VALUE OF HISTOPATHOLOGICAL GRADING SYSTEMS IN LOWER LIP AND ORAL TONGUE SQUAMOUS CELL CARCINOMA

Resumo: Objective: To evaluate the prognostic value of three histopathological grading systems in cases of lower lip (LLSCC) and oral tongue (OTSCC) squamous cell carcinoma. Study Design: Sixty-two OTSCC cases and 69 LLSCC cases were included in this retrospective study. The cases were analyzed using the histopathological grading systems proposed by Almangush et al. (2015), van Pelt et al. (2018), and Dourado et al. (2020). The relationship between clinicopathological parameters and patient prognosis was analyzed based on overall survival (OS) and disease-free survival (DFS). Results: In OTSCC cases, T3 and T4 tumors were significantly associated with ≥5 tumor buds (p=0.025) and high-risk BD score (p=0.01). In LLSCC cases, tumor budding was significantly associated with survival (OS, p=0.001; DFS, p=0.004), BD score (OS and DFS, p<0.001), and TSR/tumor budding (OS, p=0.002; DFS, p=0.012). Invasion depth was also associated with OS in LLSCC (p=0.019). In LLSCC, multivariate analysis revealed that tumor budding remained significantly associated with OS (p=0.04) and BD risk score with DFS (p=0.011), demonstrating an independent prognostic value. Conclusion: The results showed an association of the BD grading system with clinicopathological parameters of aggressiveness in OTSCC, as well as the prognostic value of the BD and TSR/tumor budding systems in LLSCC.
