Título: PROLIFERATIVE VERRUCOUS LEUKOPLAKIA AND ORAL LICHEN PLANUS. A DIAGNOSTIC CONUNDRUM. A MULTICENTRIC STUDY

Resumo: OBJECTIVE: To characterize a subset of patients who meet diagnostic criteria for Oral Lichen Planus (OLP) or oral lichenoid lesions (OLL) and who later developed white plaques fulfilling diagnostic criteria for Proliferative Verrucous Leukoplakia (PVL).
STUDY DESIGN: A retrospective study from two diagnostic centres including patients with OLP/OLL-PVL. Clinical-pathological and evolutionary variables were analysed.
RESULTS: 24 patients with a mean age of 62 years were included. 75% of them were women. Tobacco and alcohol consumption was minimal. All patients presented with OLP/OLL prior to PVL and in some both conditions persisted concurrently. The pattern of OLP/OLL was bilateral in 58% of the cases and the buccal mucosa was the most frequently affected area (79.2%). The PVL pattern was a single extensive lesion (> 3cm) in 58% of cases, gingiva being the most frequent location (45.8%). The average follow-up period was 5 years and malignant transformation (MT) was observed in 4/24 cases (16.7%).
CONCLUSION: OLP/OLL could lead to misdiagnosis of PVL. This subgroup presents a MT rate with a low proportion considering what is usually reported. This phenomenon could represent a subtype with a lichenoid appearance preceding PVL development, with a different biological behaviour that is not well-characterized in the literature.
