Resumo: Bisphosphonates (BPs) are effective antiresorptive drugs in the treatment of cancer-related conditions, including hypercalcemia of malignancy, spinal cord compression, and pathologic fractures associated with bone metastases. BPs inhibit bone resorption causing osteoclastic cell apoptosis and have been linked to drug-related osteonecrosis of the jaw (MRONJ). A 71-year-old man with metastatic prostate cancer and treatment with zoledronic acid for 32 months, complained of inflammation in the posterior mandible associated with the dental implant. Intraoral examination revealed extensive bone exposure in the mandible and purulent discharge. Computed tomography showed a hypodense area with ill-defined margins and disruption of the vestibular cortex. Antibiotics and photodynamic therapy were used to control pain and infection. After curettage of the lesion and dental implantation, the microscopic examination revealed mature lamellar bone with absence of osteocytes and intense inflammatory infiltrate. The diagnosis of MRONJ was given. The patient remains under clinical and radiographic follow-up for 6 months.
