Resumo: Fanconi Anemia (FA) is a rare genetic disease that affects the bone marrow and increases the risk of developing cancer, especially oral squamous cell carcinoma. This is a 36-year-old female patient with Fanconi Anemia who presents with hypothenar face, café-au-lait spots, pancytopenia, and multiple leukoplakias in the oral mucosa. Upon initial clinical examination, white plaques were found on the hard and soft palate with variable thickness and appearance, and incisional biopsies were performed in both sites, diagnosing hyperkeratosis with mild and moderate epithelial dysplasia, respectively. After three months, the patient presented with new diffuse lesions in the hard and soft palate, tongue, and buccal mucosa associated with erythematous areas. New biopsies on the hard and soft palate showed moderate and severe epithelial dysplasia respectively. The rapid change in the clinical appearance of the lesions emphasizes the importance of monitoring these patients aiming for early detection of oral squamous cell carcinoma.
