Resumo: Brown spiders of the genus Loxosceles are found on all continents of the world and in South America, accidents caused by this spider are considered a public health problem. Its bite can cause local dermonecrotic lesions, edema, hemorrhage, hemolysis and acute renal failure; the clinical picture of these symptoms is known as loxoscelism. The difficulty of diagnosing loxoscelism is high because it has more than 40 differential diagnoses, contributing to an underestimated record. This study aimed to report a rare clinical case of loxoscelism in a 18-year-old male patient who initially presented with a huge lip edema that was treated as an allergic reaction. However, as the edema progressed to the submandibular area and extensive lip ulceration occurred, the diagnosis of loxoscelism was stablished and anti-arachnid serum was administered. Although the patient experienced extensive lower lip necrosis, the lesion progressed to spontaneous healing five months later.
