Resumo: A 17-year-old male patient attended with extra-oral swelling on the right side of the face, associated with intra-oral swelling and ulceration in the jugal mucosa with intense pain. The initial clinical diagnosis was acute periapical abscess associated with soft tissue abscess and initial treatment was performed with Clavulin BD and dexamethasone 4mg. After clinical and radiographic examinations, the initial diagnosis was discarded because the panoramic radiography showed normal features. Serological tests of FTA-ABS and HIV were requested, both were positive and the Anti-HIV showed a viral load of 27000. In the biopsy of the intraoral lesion, an increase in the number of blood vessels and the presence of intense chronic inflammatory reaction with perivascular pattern were observed in lamina propria. The patient was referred to the Basic Health Unit for continued treatment and treatment with penicillin for syphilis, in addition to continuous monitoring of the oral lesion until complete healing.
