Resumo: Syphilis is a sexually transmitted disease caused by the spirochete Treponema pallidum. Commonly presenting manifestations of lesions in the mouth. The objective of this work is to report a case of oral primary lesion due to syphilis presenting symptoms with pain in addition to the symptomatic aspect of the treatment that the patient presented. Female patient, white, 26 years old, attended the clinic with the presence of a single lesion on the lip, left side, ulcerated, presenting spontaneous pain, with a central black area. Serological tests were requested, such as Anti-HIV, complete blood count and creatinine. She was HIV negative, VDRL and FTA-ABS positive, confirming the diagnosis of Syphilis. The choice of treatment was intramuscular benzathine penicillin G, which presented a skin reaction on the body with reddish papules, discomfort, pain and itching at night. After one week there was total regression of the lesion.
