Resumo: Glandular Odontogenic Cyst (GOC) is a rare developmental cyst, comprising 0.2% of the jaws lesions. We report a case of a 68-year-old female, presenting a painless swelling in the right maxilla. Cone beam computed tomography revealed a large hypodense lesion, expanding into the right maxillary sinus and nasal fossa, causing buccal cortical perforation. An incisional biopsy revealed pathological cavity lined by odontogenic epithelium, presenting duct-like structures, goblet cells, and apocrine metaplasia, surrounded by a fibrous capsule. Immunohistochemical analysis showed positivity for AE1/AE3, CK7, CK14, Ck19, and p63. Weak positivity for ki-67 was also observed. The diagnosis was GOC. Cystic marsupialization was performed, and the lesion was under remission after six months follow-up. COGs in maxilla are extremely rare and because of its aggressive clinical behavior and overlapping histopathologic findings with central mucoepidermoid carcinoma, the diagnosis can be challenging. Herein, we provide a discussion of the histopathological criteria of differential diagnosis.
