Resumo: Odontogenic Keratocyst (OKC) consists of a peculiar entity due to its cystic and also neoplastic characteristics, commonly presenting an aggressive behavior and high recurrence rate, which challenges the accuracy of a therapeutic approach. It is more frequent in males, and the mandible is the most common site. 30 years-old female patient was admitted with facial swelling, associated with dysphagia, dyslalia, and other infectious signs. The patient received a CT scan, which revealed a mandibular OKC, with cortical bone perforation in association with submandibular and lateral pharyngeal space collections. The patient was taken to surgery, it was performed drainage, curettage, and biopsy. The sample was referred to histopathological analysis, confirming the OKC diagnosis. Then, the treatment proposed was cyst enucleation followed by intra-lesional administration of 5-fluorouracil. The patient is currently on a twelve-month follow-up, in which the expected bony remodeling is being monitored.
