Resumo: Cemento-Ossifying Fibroma (COF) is an uncommon lesion that has clinicopathological similarities with other benign fibro-osseous lesions of the jaws (BFOL). The current case describes a mild asymptomatic swelling in the left lingual paramedian region covered by normal mucosa, observed during a routine examination in a 48-year-old female. Imaginologic exams revealed a circumscribed mixed lesion affecting the alveolar processes and periapical region of teeth 31, 32, and 33. The clinical diagnosis of BFOL was established, and an incisional biopsy was performed. Histopathological analysis revealed spindle cell proliferation in fibrous stroma, associated with trabecular and psamomatoid mineralized material. The diagnosis was COF. Complete removal of the lesion was performed followed by curettage. The patient is under a 2-years follow-up, without recurrence. In this work, we provide a discussion of the histogenesis and clinicopathological criteria of differential diagnosis of COF with other BFOLs to contribute to a better understanding of these entities.
