Resumo: Fibro-osseous lesions (FOLs) are characterized by the replacement of normal bone by fibrous and mineralized tissue. A 33-year-old female patient sought care due to recurrent facial pain. She had undergone orthognathic surgery 10 years before and underwent treatments for TMD, with several TMJ infiltrations. In the clinical evaluation, there were no signs of TMD. A maxillary swelling on the vestibular fundus was noted in an oral examination. It was performed a CT-Scan, that revealed a poorly delimited lesion with central mineralization, involving the lateral and posterior wall of the maxillary sinus, associated with bone fixation material, suggestive of an invasive FOL. Surgery was performed for biopsy, however, a fibrous capsule was observed, involving a gauze, with central calcification. The sample was taken for histopathological analysis, confirming the diagnosis of normal sinus disease and a foreign body. The patient is in a 6-month follow-up, without pain and sinusitis recurrence.
