Resumo: Condyloma acuminatum (CA) is a papillomatous lesion associated with the human papillomavirus (HPV) and usually transmitted sexually. CA appears as solitary or multiple developing at a site of sexual contact or trauma. Differential diagnosis of CA includes verruca vulgaris, squamous papilloma and focal epithelial hyperplasia. A 57-year-old male patient, prison inmate, complaining of an asymptomatic lesion under the tongue with 5 months of evolution. Clinical examination revealed a pedunculated lesion on the lingual frenulum, with a warty surface and an approximate size of 1.5 cm. After surgical excision of the lesion, microscopic examination revealed parakeratotic papillary proliferation of squamous epithelium with prominent acanthosis, koilocytosis and mononuclear inflammatory infiltration. The final diagnosis of CA was obtained. After 6 months of follow-up, the patient had no recurrences.
