Resumo: A 79-year-old patient sought treatment complaining of a slow-growing lump present at the lip. Oral examination revealed a 12x9x8mm nodular lesion with a uniform surface and fibrous consistency in the left lower lip mucosa. The medical history included a previous treatment of basal cell carcinoma located in the skin corresponding site of the current lesion. The patient detected the growing nodule approximately three years ago. The subject avoided going after treatment, afraid of cancer recurrence, since the lesion was at the site previously submitted to an extra-oral cancer resection. The differential diagnosis included: mucocele, fibroma, or foreign-body reaction. An excisional biopsy revealed, macroscopically, a surgical suture inside the lesion area. Histopathological examination showed multinucleated giant cells associated with circular chronic inflammatory cell infiltrate, characteristic of an unspecific chronic inflammatory process. The final diagnosis was a foreign-body reaction to the surgical suture. Six months after surgery, there was a complete recovery.
