Título: Inferior alveolar nerve paresthesia as the first sign of Multiple Myeloma - case report

Resumo: Multiple myeloma (MM) is a neoplastic proliferation of plasma cells with systemic repercussion and osteolytic lesions - that may cause inferior alveolar nerve (IAN) paresthesia, once located in the mandible. Therefore, we report a case of a 52-year-old female that was referred for evaluation of a periapical lesion and IAN paresthesia. Extraoral physical exam showed mild swelling on the right hemiface. Intraorally, there was no alteration. A panoramic radiography revealed an ill-defined radiolucency located in the right mandibular ramus. Blood tests showed the presence of M-protein in the serum and hypocalcemia. An incisional biopsy was performed under local anesthesia and histological and immunohistochemical diagnosis was of Plasmacytoma. The patient was referred to the hematologist to investigate other lesions and a costal arch biopsy also diagnosed plasma cell tumor. Thus, MM diagnosis was established. Dentists should be aware of MM as oral manifestations could be the first signs of the disease.
