Resumo: Cellular neurothekeomas (CN) are benign cutaneous tumors of uncertain histogenesis that rarely affect the oral cavity. An otherwise healthy 62-year-old woman was referred for evaluation of a painless swelling in the floor of the mouth of 6 months duration. Clinically, the lesion was firm and presented a pale multilobular surface. An excisional biopsy was performed, and gross examination revealed a myxoid soft tissue specimen. Histopathologic analysis showed nests of spindle cells arranged in a lobulated architecture separated by scarce hyaline collagen within a predominantly myxoid stroma. The cells presented scant eosinophilic cytoplasm with occasional nuclear pseudoinclusions. Tumor cells were positive for CD63, NSE, and vimentin, and negative for S100, SMA, and CD34. The final diagnosis was CN. The patient remains under follow-up and shows no signs of recurrence. Clinicians and oral pathologists should consider CN as a differential diagnosis when evaluating myxoid-rich lesions with lobulated architecture and positivity for CD63.
