Resumo: A 52-year-old male, non-smoker, sought a dermatologist complaining of a lesion on the left lower lip, asymptomatic and rapidly growing within 5 months. An incisional biopsy was performed, obtaining a brown to blackened fragment measuring 0.4 x 0.2 cm. The diagnostic hypotheses were paracoccidioidomycosis, american cutaneous leishmaniasis and squamous cell carcinoma. Histopathologically, the diagnosis was american cutaneous leishmaniasis. Patient underwent treatment, however, with no resolution of the lesion. Then, he was referred to an oral and maxillofacial dentist to evaluate the area, which on clinical examination, identified a crusted lesion, pale yellow to red coloration, with approximately 2 cm in its largest diameter. Another incisional biopsy was performed, obtaining a 0.4 x 0.2 x 0.2 cm specimen, that on anatomopathological examination, revealed a poorly differentiated ulcerated squamous cell carcinoma, with involvement of the lateral surgical margins and without angiolymphatic invasion. The lesion was surgically removed and patient remains under follow-up.
