Título: ORAL NODULES AS THE FIRST MANIFESTATION OF SARCOIDOSIS: A CASE REPORT

Resumo: To describe a case of sarcoidosis in an 85-year-old white woman with lesions in oral cavity and on the skin. Both lesions were described as asymptomatic, consistent, and well-defined, nodules adhered to underlining tissues. An excisional biopsy from the oral lesions were taken and submitted to histopathological examination. This analysis revealed the presence of focal, compact, lobular aggregations of activated macrophages, epithelioid histiocytes, Langerhans cells, and lymphocytes. In focal areas, starry inclusions (asteroid bodies) could be evidenced inside the cytoplasm of multinucleated cells. Special stains do not support the presence of infectious agents. Complementary exams were requested, and revealed hypercalcemia, high serum angiotensin converting enzyme, and high alkaline phosphatase levels. Together, the clinic and pathological data support the final diagnosis of Sarcoidosis. This patient was periodically monitored by 3 years. Oral lesions may be the first or the only sign of the illness ad it can disappear between 2-5 years.
